ROLE OF THE LIVV PLATFORM LIVV Care LLC operates a technology platform that facilitates connections between users and independent, state-licensed healthcare providers ("Providers") and licensed pharmacies. LIVV does not: - Practice medicine - Practice pharmacy - Provide medical advice - Control clinical decision-making - Diagnose medical conditions - Determine whether prescriptions are issued - Manufacture, compound, dispense, or ship medications Medical services are provided solely by independent healthcare providers, and medications are dispensed by licensed pharmacies operating independently of LIVV. LIVV does not control the professional judgment of healthcare providers and is not responsible for clinical decisions made by providers. Use of the Platform does not create a provider-patient relationship with LIVV. Healthcare providers and pharmacies are responsible for maintaining medical records and complying with applicable healthcare laws, including HIPAA where applicable. LIVV may operate as a technology platform or service provider supporting healthcare providers and may, in certain circumstances, act as a business associate under applicable healthcare privacy laws. Medication Images and Illustrations Images or illustrations of medications, packaging, or labels displayed on the Platform are provided for informational and illustrative purposes only. Any medication that may be prescribed through a healthcare provider using the Platform will be compounded or dispensed by a licensed pharmacy in accordance with applicable pharmacy laws and regulations. The appearance, labeling, packaging, and pharmacy identification on the medication you receive may differ from any images displayed on the Platform. 2. INFORMATION WE COLLECT We may collect several categories of personal information.
